This section provides you with the opportunity to see the documents you will complete and sign once you choose to be represented by us. Please feel free to review these forms. Included are the Authority to Represent, Statement of Clients Rights, Liability Claim Summary, Release for Accident Request and Medical Authorization. In the event that you would like to sign up with us through the mail, once we have agreed to represent you, then you can download and print these forms. After you have completed and signed everything, you can then mail them to our office. It is necessary that we receive documents with original signatures in order to represent you.
